I supply the DVLA D4 medical form, so you don’t need to worry about getting hold of one. You will need a D2 form to renew your licence.

I will need details of any hospital treatment or investigations you have had in the past together with the dates when you were seen and the details of any hospitals you attended.

If you are taking medication please bring it all with you. The DVLA do need details of doses, etc.

I need a fresh urine sample, so please try to hang on until you get here!

If you use either spectacles or contact lenses for driving you must bring your latest optician’s

prescription with you. Don’t forget your spectacles too! I do need to test your vision with and without them. If you wear contact lenses please bring along anything you need so that they can be removed for the eye test.

Payment can be either cash or a cheque. Sorry, I do not have facilities for card payments. I will provide you with a receipt.
